Appointments
Our staff is available to schedule your appointment during our normal business hours. Please contact (719) 839-1923 to set up your visit or use our convenient on-line scheduling option via the Patient Portal.
Cancellations
We realize that situations may arise where you need to cancel or reschedule your appointment. We ask that you kindly give us 24 hours’ notice by calling our office.
Office Visits
Please arrive early enough before your office visit in order to allow enough time to update your medical insurance information and process your payment as required. Please ensure that you bring all pertinent information as well as any medications you are currently taking in their bottles.
Prescriptions
We prefer to handle all prescription refills during normal office hours only. If you have not been seen in 6 months or more, you must schedule an appointment in order for your medication refill to be completed. Some medical conditions need to be monitored more frequently and require quarterly visits.
Medical Questions
During regular office hours, our staff is available to address any medical concerns you may have. Our Provider is on call after normal office hours to handle emergency medical needs. Please call during normal office hours for routine questions/concerns.
FMLA Forms
If you are planning time off work for maternity leave or surgery, your employer may require you to have FMLA or other forms filled out by your provider. Our office staff will be happy to help complete any needed paper work. There will be a $25 fee due at the time of completion of these forms. Please allow at least 10 business days for completion.
Insurance & Billing
We do require that all patients meet their financial obligations at the time of service. All patients are required to review and participate in our billing policy. If you have questions regarding our billing policies, please contact our office at 719-839-1923.
