Your Annual Wellness Exam is one of the most important steps you can take for your health. Most insurance plans cover this visit at no cost to you. Call us to learn more.

When you need a specialist, we coordinate the referral and transfer all relevant records so your care stays connected and nothing falls through the gaps.
Same-Day Care Available
From minor injuries to routine monitoring visits, we offer same-day and next-day appointments for existing patients so you can be seen quickly by a provider who knows your history.
When an appointment is missed without notice, another patient loses access to care. We appreciate your consideration for everyone in our community.
A $25.00 fee applies to appointments cancelled with less than 24 hours notice or not cancelled at all. This fee is not covered by insurance and is due before your next visit.
Patient financial obligations are due at the time of service. No-show and late cancellation fees are billed directly to the patient and are not covered by insurance.
Three missed appointments within a 12-month period may result in discharge from our practice. We appreciate your respect for our team and your fellow patients.

Please arrive 20 minutes early to allow time to update your insurance information and process payment. Bring a valid photo ID, your insurance card, and all current medications in their original bottles.
Our team returns all calls within 24 business hours during regular office hours. For routine questions, please call during normal hours. For after-hours emergencies, our provider is on call and available to assist.
We require all patients to meet their financial obligations before leaving the clinic. For detailed billing questions or to review our policy, call us or visit the office.
We ask for at least 24 hours notice if you need to cancel or reschedule. This helps us offer your time slot to another patient who needs care. A $25.00 fee applies to no-shows and late cancellations.
Prescription refills are processed during regular office hours only. If you have not been seen in the past 6 months, an appointment is required before your refill can be completed. Some conditions require more frequent monitoring and quarterly visits.
Yes, we complete FMLA forms and other medical leave paperwork. A $25 fee is due at the time of completion. Please allow at least 10 business days for your forms to be processed.
