florissantfm.com is an information only portal for small, family medicine, medical practice office located in Florissant, Colorado. Our website will receive the information that you provide to our Contact Us page only. We do not use cookies to track locations or your personal informaiton. Any information you provide in the Contact Us web-form is emailed directly to our office and managed internally. We promise not to sell your information or provide it to any third-pary that you have not already specific in your Patient Portal account.

• Fulfill your requests for appointment or services and for related medical needs, such as scheduled appointments, referrals, medication refills or prescription updates & deliveries;

• Determine if you are eligible for certain medical services, devices, and continued medical services;  i.e. insurance validation inquiries  

• Provide you with additional information (electronically) that may be of interest, such as patient education and follow-up informtion for future appointments

• Periodic text message or email reminders that you have given us permission to send to your relevant email address or mobile device numbers
